Сustom Healthcare API Integration and Development
Standards, Architecture, Security Requirements
IT Craft specializes in development and integrating third-party APIs into custom healthcare platforms. We connect EHRs, labs, billing systems, and telehealth platforms through secure, HIPAA-compliant APIs.
Core Standards & Data Formats for Development & Integration
Healthcare systems exchange data in several formats, and most of integration projects involve more than one. Here is where each standard usually shows up in our projects:
| Standard | What it carries | How we use it |
|---|---|---|
| FHIR (R4 and newer) | Modular clinical resources over REST, JSON or XML | Default for new APIs and patient-facing apps. US regulations require it for patient access APIs |
| HL7 v2, v3 and CDA | ADT events, lab orders and results, clinical documents | We connect to existing hospital feeds directly or expose them as FHIR resources |
| DICOM / DICOMweb | Medical images and PACS data | Query and retrieval of X-ray, CT and MRI studies over HTTP |
| X12 / EDI | Eligibility (270/271), claims (837), remittance (835), prior authorization (278) | Billing, clearinghouse and payer integrations |
| SNOMED CT, LOINC, ICD-10 / ICD-11 | Clinical terminologies | Mapping incoming data so every system reads the same record the same way |
Key Architecture & Security Requirements
Healthcare APIs carry protected health information (PHI), so security decisions are made in first sprint.
Access control starts with OAuth 2.0 and SMART on FHIR scopes. They limit data by clinical role and launch third-party apps in the correct patient context. OpenID Connect handles sign-in for patients and clinicians, and JWT tokens carry the session. Requests usually go through an API gateway. Data is encrypted with TLS in transit and also at rest, role-based access control (RBAC) decides who can see what, and audit logs record who accessed which record and when.
On compliance side IT Craft works with HIPAA in the USA, PIPEDA in Canada, and GDPR in the EU. Every vendor that processes PHI signs a Business Associate Agreement (BAA). For analytics, managed platforms like the Google Cloud Healthcare API or a custom FHIR facade connect legacy data silos with BigQuery and NLP engines.
IT Craft’s Services for Healthcare API
IT Craft assists in specific healthcare API project stages as well as providing full-cycle API development, integration and support. Our healthcare API services include:
Our Healthcare API Integration Process
We handle healthcare API delivery end-to-end, from the first system audit to post-launch monitoring. Here are main steps we follow:
-
Discovery and system audit
We map your systems, workflows, and data sources and check which vendor APIs are available. Compliance gaps get flagged before design starts.
-
Architecture and standards
Each connection gets a standard (FHIR, HL7 v2, X12, or a custom API). It also gets a pattern – integration engine, API gateway, or FHIR facade.
-
Data mapping
Our engineers define data objects, transformation rules, and terminology mapping.
-
Development and testing
The team builds endpoints and authentication. Then come checks in vendor sandboxes and FHIR conformance tests.
-
Vendor certification and go-live
We handle EHR marketplace approval and security reviews, for example Epic Showroom, and deploy to the production.
-
Monitoring and support
Performance and error tracking, version management and updates as vendor APIs and regulations change.
Telemedicine Web API Standards
| Workflow | Standards |
|---|---|
| Video and audio visits | WebRTC for real-time audio and video in browsers and mobile apps, no plugins. It carries no clinical data, so visit records still go to the EHR through FHIR |
| Scheduling and visit records | FHIR appointment and encounter data |
| E-prescriptions | FHIR resources in the patient record |
| Remote image review | DICOMweb |
| Remote patient monitoring | FHIR observations from devices and wearables |
| Sign-in and app launch | SMART on FHIR, OpenID Connect |
| Accessibility | WCAG: screen reader support, keyboard navigation, color contrast, captions for video |
Challenges & Solutions
“We need to connect to Epic, Cerner, and other EHRs, but the vendor approval process is a maze.”
We have done EHR vendor onboarding many times. We handle app registration, sandbox testing, security reviews and go-live requirements for Epic Showroom, Oracle Health (Cerner) Code, athenahealth Marketplace, and other platforms. Your team can focus on product instead of the paperwork.
“Our data arrives as HL7 v2, FHIR, CSV, and scanned PDFs, and none of it matches.”
We turn fragmented healthcare data into one consistent model. HL7 v2 messages, FHIR R4 resources, CCDs, X12 EDI, and legacy flat files all get mapped and normalized with standard terminologies (LOINC, SNOMED CT, ICD-10, RxNorm). We make sure that every system that you connect then sees the same patient.
“Our integration worked perfectly in the sandbox, then broke as soon as real hospitals connected.”
Every health system configures its EHR differently. Sandboxes do not show that. We build for the real-world variation with tolerant parsing, per-site configuration, message queueing and retry logic, and proactive monitoring. That monitoring catches failed transactions before clinicians are noticing missing data.
“Our APIs expose patient data, and we can’t afford a security or compliance gap.”
API security is built from the start into the architecture. We implement HIPAA-compliant data flows, SMART on FHIR authorization, scoped OAuth 2.0 access, and consent management. Encryption in transit and at rest is part of it, along with detailed audit logs. Every API call is traceable and review-ready.
“We have to integrate with labs, pharmacies, payers, and devices, and each has its own standard.”
Multi-party integration is our everyday work. We connect lab information systems, e-prescribing and pharmacy networks, insurance eligibility and claims (X12 270/271, 837/835) and prior authorization services. Wearables and connected medical devices, too. It all run through one well-structured API layer.
“Our point-to-point integrations have become impossible to maintain.”
You do not need to rip everything out. We audit your existing interfaces and consolidate them behind interface engine or API gateway. Then come versioning and documentation. Fragile custom connections get replaced step by step, without interrupting the clinical workflows that depend on them.
“Interoperability regulations keep changing, and we can’t keep rebuilding our integrations.”
We design for change, not one-time compliance. Our modular, standards-based architecture follows current requirements such as the Cures Act information-blocking rules, USCDI, and CMS interoperability and prior authorization APIs. When those rules are changing, it adapts without a full rewrite.
Our Successful Project
Healthcare API Development for Arctrieval
IT Craft built a secure healthcare SaaS platform for Arctrieval that lets U.S. personal injury law firms manage medical records requests and certified correspondence in one system. Built with Angular and ASP.NET Core, it integrates with leading case and practice management systems and supports e-signing and email, fax, and postal delivery. It now processes tens of thousands of HIPAA-aligned requests daily.
